Population ageing is accompanied by rising numbers of elderly people with multimorbidity requiring high-quality chronic care; however, the long-term, high-intensity care-giving evidentially imposes an immense burden on the family caregivers, and there is great demand for improved chronic care models. Hence, the Guided Care Caregiver Interview encompasses a 125-question survey for the detailed interrogation of a caregiver, which creates the basis for the Guided Care Program for Families and Friends (GCPFF), a component of Guided Care (GC) focusing on assuring the well-being of both chronic disease patients and their caregivers. Advantages include:

Technical Details:

Johns Hopkins researchers have designed a Guided Care Caregiver Interview to gather comprehensive information about the health, employment, and sociodemographic status and the specific care-giving tasks of a family caregiver, permitting to assess the effects of GC on the caregiver and improve chronic care. Family care-giving critically contributes to the high-quality care of chronically ill patients; however, to date, interventions to support caregivers have focused on providing resources and emotional support, and novel strategies to better assist caregivers and improve the quality of chronic care are required. Therefore, this creation provides a Guided Care Caregiver Interview fundamental for GCPFF, a new program that systematically supports family caregivers while simultaneously providing a patient-centered chronic care. The 125-question survey investigates the caregivers sociodemographic characteristics, employment, health and the type of assistance provided to a GC patient. Together, this comprehensive survey allows evaluating the effects of practicing GC on the primary family caregiver and enables the customized application of intervention strategies to support the caregiver, thereby ultimately increasing the quality of chronic disease management.
