Johns Hopkins researchers have created a Guided Care Caregiver Assessment Form that allows a GC nurse to gather important information about a caregiver and the care-giving-related challenges, permitting the provision of supportive information and training, thereby assisting the caregiver and improving the quality of chronic care.
Family caregivers play a crucial role in the chronic disease management of older adults; however, to date, interventions to support caregivers have focused on providing resources and emotional support, and new strategies to better assist caregivers are required. Hence, this creation provides a Guided Care Caregiver Assessment Form, essential for the Guided Care Program for Families and Friends (GCPFF), which systematically supports family caregivers while simultaneously providing a patient-centered chronic care. The 8-page survey comprehensively gathers information about the caregivers sociological and demographic characteristics, employment, health and the type of assistance provided to a GC patient. Further, the individual needs for information, training, and healthcare services are interrogated. Together, this survey presents a valuable new instrument to evaluate the family caregivers unique needs and provide personalized assistance, thereby assuring the well-being of the caregiver and increasing the quality of chronic disease management.
